In Act one after the grove, which from what i remember i ended up spending time with karlach, no sex though and i didnt specifiacally reject shadow either. You can just reload a save to watch the scene but nothing changes relationship wise with shadowheart if you skip out on it. I had to watch the scene with both in clown makeup cause I didn't know how to get rid of it. Resting was the only thing that cleared it.
