Current USA *medical* heme/onc (HO) fellow here. I'll make a note that you can take care of a lot of oncology patients (not heme) via other specialties: radiation oncology, surgical oncology, dermatology, the various surgical subspecialties, etc. so worth thinking about. I'll focus on the clinical side of things.* Consider whether you can have a longer and more fulfilling career as a hospitalist or as hem-onc. For example, if you are neutral about being a hospitalist and quit after a few years vs. if love hem-onc and work for decades, then it'd be worth it to do hem-onc. Med/onc is the chemo deciders. Longer term relationship with patients. Ability to sub-specialize (breast, GI, etc).Cards, GI, and Pulm/CC are probably the most competitive of the IM fellowships. You make good money, but have to really work for it. Heme/Onc is competitive, as well.
