I had the same problem on my Gen 2. If you remove the dust cover you can watch the action. Dry fire, keeping the trigger depressed. Rack the bolt carrier and watch closely while releasing the trigger. I had to file a tiny bit of material from the underside of the hook part of the disconnector. I polished it up and it works perfectly every time now.

It can be added to the Gen 1, everything else I have found pointless. With the lightest AR stock (B5 CAR15 replica) and Occam Defense sights the Gen 2 still is 2 ounces heavier than the Gen 1.
